Five Rivers Care, founded by like-minded community members in the Peel region, draws its name from the Kimberly region of Western Australia. The rivers (Ord, Forrest, King, Durack, and Pentecost) converge and flow into the Cambridge Gulf. This convergence symbolizes our holistic care approach, where individuals rely on diverse services to meet their unique care requirements.
Our vision is to combine many services together to obtain the one objective – high quality of living for all!

We accept referrals from participants and their families, NDIS Local Area Coordinators, Support Coordinators, Planners and other care organizations. Your support and capacity are our priority.
